Dear Supervisor,
As you may be aware, an employee who you supervise has requested a COVID-related workplace accommodation due to a personal medical condition. We are currently working to assist the District in reviewing all requests. To do so we need some additional information from you on if this employee can do their entire job from home.
To support the District to evaluate requests, we respectfully request that you complete the below survey as soon as possible.
Please note, we are asking if this employee can do their whole job from home when the campus is fully reopened with in-person student services offered as they were pre-COVID.
