As you may be aware, an employee who you supervise has requested a COVID-related work from home accommodation due to a personal medical condition. We are currently working to assist Metro in reviewing this request. To do so we need some additional information from you on if this employee can do their entire job from home.
To support Metro to evaluate requests, we respectfully request that you complete the below survey as soon as possible.
